I waited a few months after I sent the little slip in to IBM after getting
my 765D. Then I called up IBM support one night. I was told they had
someone else doing it -- it did seem like another company. I was given
their number and called this other bunch directly the next day. I got the
name plate 2 weeks later. More than a month latter, I got the one I asked
for on the little slip I mailed.

I'm not sure who screwed up, but it seems that the bunch actually making
the name plates can deliver them in a somewhat timely manner. I may still
have some contact info on them at home.
