Well, its gone away.

Now before you think I'm crazy... The white pixel showed up on top of any
application I had in focus and use at the time (presuming it was a dark
enough background to see it). I finally at some point moved my cursor to
that pixel, and when I was precisely on it, the cursor changed into a
scrollbar cursor, just like what you get when you click the blue scroll
middle button above the trackpad. When I moved off of the pixel, the
cursor changed back. This was repeatable.

Nothing open seemed responsible. A reboot fixed it.
