For a long time I thought it wasn't possible to enable the Expanded Desktop feature using Windows 2000 on a ThinkPad without a dock.
However, I did manage to get it to work today, and for everyone's edification here's how:

Implementing Windows 2000 Expanded Desktop
on the IBM R40 ThinkPad 2897.

1. Install the latest ATI video driver from IBM's update site
The driver used here is:
http://www-307.ibm.com/pc/support/site.wss/document.do?lndocid=MIGR-41910
IBM ThinkPad July 4, 2004
Display Driver (Mobility-RADEON/FIREGL Series) for Windows 2000
Version 7.983.2-040513a-015547C-IBM

2. make sure the ATI icon is displayed on the taskbar with:
a. right-click on desktop to get the display properties
b. click on "settings"
c. click on "advanced"
d. click on "options"
e. make sure there is a check mark next to "Show ATI icon on taskbar"
f. Click "OK" until the display properties closes

3. Plug your external monitor into the video port on
   the back of the ThinkPad

4. If both the LCD and monitor do not display your desktop,
type ALT-F7, until they both do.

5. right-click on the ATI icon on the taskbar
6. Select "displays"
7. Select "extend right" or "extend down" on one of your displays.
8. You should now have a desktop that covers both of the displays.

Note: It may be necessary to install the .INF driver file
      for your external monitor
